Scope of Appointment – Required Before We Meet
Before we can discuss Medicare Advantage or Part D Prescription Drug Plans, we're required by Medicare to have a Scope of Appointment (SOA) form completed. This form ensures that you are informed and in control of what topics will be discussed during your appointment.

This rule protects you by documenting the specific topics you’d like to cover — and helps us stay compliant with CMS (Centers for Medicare & Medicaid Services) guidelines.
